Become an Athlete

Athlete Application for Participation / Medical Form - You must have a valid Athlete Medical form on file before participating in any Special Olympics Oregon events, practices or competitions. A new medical form must be completed every three years.

During times of COVID-19 - Please make sure that you complete your Medical Form and the COVID-19 Waiver and send back to your Local Program.  These documents will be valid for three years.

Sports Schedule



We will have basketball training this season and will be attending the regional competition.

Start Date:  January 16

Practice Days:  Mondays and Wednesdays

Practice Times:  6:30 PM - 8:00 PM

Location: Learning Center Gym, Coquille Tribe Reservation, 591 Miluk Dr, Coos Bay, OR 97420

ATHLETES - The medical deadline for this season is January 23rd.  We must have your completed medical by that date in order for you to train and compete.

COACHES -- We need more coaches!  Please see Class A Volunteer information at the bottom of the page!

Your Local Program Team

We are recruiting volunteers for the Local Program Management Team!

Local Program managers plan, organize and coordinate programs and services for the athletes and volunteers withtin their local program so that athletes can train and compete in sports and enjoy the camaraderie of Special Olympics Oregon!

Class A Volunteer

All volunteers who have close contact with athletes or who handle the financial assets of Special Olympics Oregon must complete a Class A volunteer application. This includes coaches, chaperones, Unified Partners and Local Program Management members. Volunteers need to complete the Class A Volunteer process which includes an online application, background check and online trainings.
