Become an Athlete
Athlete Application for Participation / Medical Form - You must have a valid Athlete Medical form on file before participating in any Special Olympics Oregon events, practices or competitions. A new medical form must be completed every three years.

Become a Coach
Complete the steps listed below to become a coach in your local program:
All volunteers over 18 years of age who have close contact with Special Olympics Oregon athletes must complete a Class A volunteer application. As part of the application process, our organization will complete a background check on the applicant.
For minors applying to become an assistant coach (ages 16-17) or team helper (ages 12-15) complete the following steps:
All minor volunteers who have close contact with Special Olympics Oregon athletes must complete a Class A minor volunteer application.
Become a Unified Partner
Complete the steps listed below to become a Unified Partner in your local program:
All volunteers over 18 years of age who have close contact with Special Olympics Oregon athletes must complete a Class A volunteer application. As part of the application process, our organization will complete a background check on the applicant.
