Become an Athlete
Athlete Application for Participation / Medical Form - You must have a valid Athlete Medical form on file before participating in any Special Olympics Oregon events, practices or competitions. A new medical form must be completed every three years.
During times of COVID-19 - Please make sure that you complete your Medical Form and the COVID-19 Waiver and send back to your Local Program. These documents will be valid for three years.
START DATE: January 9
DAY and TIME: Mondays and Wednesdays ... 4:30 pm - 8:55 pm
WHERE: All teams will practice at Allen Dale Elementary School, 2320 Williams Highway, Grants Pass, 97527. Please be on time!
INDIVIDUAL SKILLS will practice from 4:30pm-5:25pm. Coaches and Assistant Coaches will be Ronnie Kendrick, Charlie Kendrick and others.
3v3 TEAMS will practice from 5:30pm-7:00pm. Coaches and Assistant Coaches will be Kathi Holmbeck, Kevin Bogart, Wayne Gamby, Bruce Mimnaugh, and others.
5v5 TEAMS will practice from 7:05pm-8:55pm. Coaches and Assistant Coaches will be Anthony Hauckes, Lorene Elzea, Mike Harvey, and others.
START DATE: January 11
DAY and TIME: Wednesdays ... 5:30 pm - 7:00 pm
WHERE:Our practices will be at Relentless Athletic Performance, 2163 NE Spalding Avenue .#14, Grants Pass, 97526.
COACHES: Janet Capetty will be our Head Coach, assisted by other knowledgeable assistant coaches and volunteers.
INFORMATION FOR ALL ATHLETES - ALL SPORTS
MEDICALS: You must have a valid medical in order to train and compete. Medical deadline is Monday, January 23.
SEASON CALENDAR: You will be given a season calendar at the first pracitce showing all the practice dates.
CLOTHES / JEWELRY:
- Please wear clean, comfortable clothes.
- Please avoid anything that might be offensive to your teammates.
- Please wear modest clothing – nothing too revealing.
- No jewelry, please.
- No black soles as they may leave marks on the gym floor. Must be athletic shoes with non-skid soles.
ABSENCES: If you need to be gone for a practice, please talk with your coach ahead of time, and fill out an Excused Absence Form.
ARE YOU SICK? If you are not feeling well, PLEASE do not come to practice. You don’t want to get your teammates and friends sick too!
COURTESY: Special Olympics is supposed to be FUN! We can do that if we are kind to each other and encourage one another. The volunteers are there to help keep you safe and keep things running smoothly. Please listen to them and follow their directions.

We DO plan to offer basketball and powerlifting this Winter season.

Class A Volunteer
All volunteers who have close contact with athletes or who handle the financial assets of Special Olympics Oregon must complete a Class A volunteer application. This includes coaches, chaperones, Unified Partners and Local Program Management members. Volunteers need to complete the Class A Volunteer process which includes an online application, background check and online trainings.
