Become an Athlete
ATHLETE APPICATION FOR PARTICIPATION / MEDICAL FORM
You must have a valid Athlete Medical Form on file before participating in any Special Olympics Oregon events, practices or competitions.
- Complete and sign all four pages of the medical form.
- Make an appointment for a physical examination with your doctor or licensed physician.
- At your doctor's appointment, have your doctor fill out and sign the Physical Examination section at the bottom of page one.
- Return this completed form to your Local Program.
- We recommend making a copy of the completed medical.
A new Medical Form must be completed every three years.
Class A Volunteer
COACH - UNIFIED PARTNER -
LOCAL PROGRAM MANAGEMENT TEAM
All volunteers who have close contact with athletes or who handle the financial assets of Special Olympics Oregon must complete a Class A volunteer application. This includes coaches, chaperones, Unified Partners and Local Program Management members. Volunteers need to complete the Class A Volunteer process which includes an online application, background check and online trainings.

SOCCER

Practices Start: Sept 19
Practice Day and Time: Tuesdays & Thursdays 6 pm - 7:30 pm
Location: Reinhart Volunteer Park, 1690 SW Webster Rd, Grants Pass
State Competition: Nov 18-19, Providence Park, Portland
SWIMMING

Practices Start: Sept 9
Practice Days, Times and Locations: Mondays -3:30 pm - 5 pm -Club Northwest, 2160 NW Vine St, Grants Pass Saturdays -10 am - 11:30 am, YMCA, 1000 Redwood Ave, Grants Pass
Regional Competition: Nov 4, Osborn Aquatics Center, Corvallis
INFORMATION FOR ALL ATHLETES - ALL SPORTS
MEDICALS: You must have a valid medical in order to train and compete. Medical deadline is Sept 4 for Fall Season.
SEASON CALENDAR: You will be given a season calendar at the first pracitce showing all the practice dates.
CLOTHES / JEWELRY:
- Please wear clean, comfortable clothes.
- Please avoid anything that might be offensive to your teammates.
- Please wear modest clothing – nothing too revealing.
- No jewelry, please.
ABSENCES: If you need to be gone for a practice, please talk with your coach ahead of time, and fill out an Excused Absence Form.
ARE YOU SICK? If you are not feeling well, PLEASE do not come to practice. You don’t want to get your teammates and friends sick too!
COURTESY: Special Olympics is supposed to be FUN! We can do that if we are kind to each other and encourage one another. The volunteers are there to help keep you safe and keep things running smoothly. Please listen to them and follow their directions.
