Become an Athlete

ATHLETE APPICATION FOR PARTICIPATION / MEDICAL FORM

You must have a valid Athlete Medical Form on file before participating in any Special Olympics Oregon events, practices or competitions.

  1. Complete and sign all four pages of the medical form.
  2. Make an appointment for a physical examination with your doctor or licensed physician.
  3. At your doctor's appointment, have your doctor fill out and sign the Physical Examination section at the bottom of page one.
  4. Return this completed form to your Local Program.
  5. We recommend making a copy of the completed medical.

A new Medical Form must be completed every three years.

Class A Volunteer

COACH - UNIFIED PARTNER -
LOCAL PROGRAM MANAGEMENT TEAM

All volunteers who have close contact with athletes or who handle the financial assets of Special Olympics Oregon must complete a Class A volunteer application. This includes coaches, chaperones, Unified Partners and Local Program Management members. Volunteers need to complete the Class A Volunteer process which includes an online application, background check and online trainings.

Click on the link below to complete the application and trainings.  The system will automatically guide you through the appropriate training modules for adults and minors.

Summer 2024 Sport Registration

AS SOON AS YOU GET THIS, PLEASE USE THE LINK,  CALL OR EMAIL TO REGISTER!  WE WILL NOT BE SENDING OUT REMINDERS!  We will send out more detailed practice info and addresses to those who registered for each sport closer to the starting date.

Coaches: If you are interested/able to help coach this winter season, and you haven’t let us know, please let us know what sport you’d like to coach.

Registration/Medical deadline will be April 10. This will also be the deadline for any medicals that need to be submitted. If you need a medical packet, let us know. Your medical is current if you have not received an email or mailed medical packet from Kristi.  If you haven’t been participating with us, contact us to see if you have a medical on file.

To register: There are three ways to register this season.

Practice schedule:

Track & Field: Sundays, South Eugene High School, 4-5:30pm

Starting: Sunday, April 21

Regional competition: June 15 at Willamette University

Softball: Tuesdays, Washington Park, Eugene between 5-8pm depending on team. Those registered for softball will get more information about times after registration closes.

Starting: Tuesday, April 23

Regional: June 23, Eugene

Bocce: Sundays, Hamlin Middle School, 2-3:30pm

Starting: Sunday, April 28

Regional: June 16 at Hamlin Middle School

Summer State Games:  All sports: July 13-14 Oregon State University, Corvallis.

We will travel to Corvallis on Friday evening so make plans to                           be available for the weekend.

Unified Intramural Basketball: Sundays, 11:30-12:30pm,  Field #3 UofO campus

Starting: April 21 to May 19.

You must be 18 years of age or older to participate.

Red Robin Restaurants are holding fundraisers for Special Oympics the first Saturday of the month with 20% of the proceeds going to our local program.  Please mention Special Olympics or bring the flyer below to our local Valley River Center Red Robin to help support us.  Every dollar helps us provide the best program for our athletes.
