LouAnne Tabada, SOOR staff, will be joining us. She gives a huge "shout out" to David and Steve McAloney as co-Local Program Coordinators, for their dedication to this program, all their hard work to bring the program back in action and expanding the sports program!

We look forward to bringing our team together so we can continue to expand our program and provide the best for all of our athletes!

You must have a valid Athlete Medical Form on file before participating in any Special Olympics Oregon events, practices or competitions.

  1. Complete and sign all four pages of the medical form.
  2. Make an appointment for a physical examination with your doctor or licensed physician.
  3. At your doctor's appointment, have your doctor fill out and sign the Physical Examination section at the bottom of page one.
  4. Return this completed form to your Local Program.
  5. We recommend making a copy of the completed medical.

A new Medical Form must be completed every three years.

Class A Volunteer

Volunteers need to complete the Class A Volunteer process which includes an online application, background check and online trainings.
